The new Hammers signing was involved from the start as Spain dominated to win Thursday's semi-final 4-1.
They recovered from the early blow of falling behind to lead at the break thanks to Marc Roca's equaliser and Mikel Oyarzabal's penalty.
Daniel Olmo added to the lead two minutes after the interval before Borja Mayoral wrapped up victory midway through the second period.
Fornals sparkled throughout, completing 90 percent of his passes, creating four chances and laying on Mayoral's goal.
Spain will face Germany in the final in Udinese's Dacia Arena on Sunday.
